Care Levels
Not all individuals fit precisely within each care level’s definition. An initial assessment and periodic assessments by the Director of Nursing Services are conducted for ongoing evaluation of how each resident’s needs are best served. The Director of Nursing Services shall determine the appropriate level of care to meet each individual’s medical, personal, and safety requirements. Some complicated medical conditions or behaviors may preclude acceptance to assisted living or memory care.
Memory Care Level Descriptions
Level 1: Designed for residents diagnosed with mild cognitive impairment. These residents need regular cuing, but little to no physical assistance with activities of daily living, including no resistance to hygiene care and no exit-seeking behavior. An example would be a resident who is no longer considered safe to live at home because of short-term memory loss and impaired safety or decision-making ability.
Level 2: Designed for residents diagnosed with moderate cognitive impairment or decline. These residents need limited physical assistance with activities of daily living, mobility, continence management, some care-resistant behaviors and/or exit-seeking that requires regular monitoring. An example would be a resident experiencing changes in behavior that impact on the safety of themselves or others.
Level 3: Designed for residents with significant cognitive impairment or decline. These residents may require an extensive amount of physical assistance with activities of daily living. An example would be a resident who may have significant physical impairments or behaviors that require frequent nursing interventions.
When Memory Care Is Not Enough
In many cases, your loved one will be able to receive end-of-life care through palliative and/or hospice support here in our Memory Care community. However, there may come a time when memory care is not enough. If your loved one requires around-the-clock nursing support by medical staff, our long-term care nursing home may be most appropriate.
